If you have a prescription from your doctor: Download and print a mail-order form by clicking the "printing mail-order forms online" link on the Forms page.   Mail the completed form and your prescription slip(s) to the address indicated on the form.

If you have questions about getting your prescriptions by mail:

If you are going to see your doctor:
Before you go to your doctor, download the physician fax form* and ask him or her to fill it out and fax it to the number indicated on the form.

Please note: Only your physician may fax the completed form to us.

*The physician fax form is an Adobe^® Portable Document Format (PDF) file. You will need to download a free copy of Adobe^® Reader^® if you do not have it already.
